સક્ષમ સત્તાની મંજુરી અનુસાર જાહેરખબર ક્રમાંક: ૨૨/૨૦૨૩-૨૪ તા:૦૪/૦૯/૨૦૨૩ મુજબ મલ્ટી પરપઝ હેલ્થ વર્કર (MPHW) ની જગ્યા માટે નીચે જણાવેલ ઉમેદવારોએ અસલ પ્રમાણપત્રોની ચકાસણી / સ્ક્રુટીની અર્થે ઉમેદવારના નામ સામે દર્શાવેલ તારીખે બીજો માળ, લેક્ચર હોલ – ૩, એન.એચ.એલ. મેડીકલ કોલેજ, એલીસબ્રિજ, અમદાવાદ. ખાતે અસલ ફોટો આઇ.ડી., સ્કુલ લિવિંગ સર્ટીફીકેટ, શૈક્ષણીક લાયકાત (સેમેસ્ટર વાઇઝ માર્કશીટ અને ડીગ્રી સહીત), અનુભવના સર્ટીફીકેટ અને આનુષાંગિક પુરાવા તથા જાતિના પ્રમાણપત્ર (૧ – ઝેરોક્ષ સેટ), અરજી રીસીપ્ટ અને કોલ લેટર સાથે હાજર રહેવા જણાવવામાં આવે છે.
**ગેરહાજર રહેલ ઉમેદવાર આગળની ભરતી પ્રક્રીયા માટે ગેરલાયક ઠરશે**
ઉમેદવારોને ઓનલાઇન અરજીમાં દર્શાવેલ વિગતોના આધારે માન્ય ગણવામાં આવેલ હોઇ, રૂબરૂ ચકાસણી સમયે અસલ પ્રમાણપત્રોમાં જો કોઇ ક્ષતિ જણાશે, તો ઉમેદવાર આગળની ભરતી પ્રક્રીયા માટે આપોઆપ રદબાતલ ઠરશે.
